The NHS Business Services Authority (NHSBSA) recently published a report on Hormone Replacement Therapy (HRT) prescriptions in England, covering the period 2015 to June 2023. The report shares fascinating insights, shedding light on the changing HRT prescription scene in England, particularly over the last two years.

Over twenty years ago, two studies - the Women’s Health Initiative (2002) and the Million Women Study (2003) - published findings that left a lasting impression. They suggested HRT may have inherent risks. The studies generated a high level of media coverage which led to a sudden halt in women using it. HRT prescriptions fell off a cliff. Subsequent generations have since grappled with menopausal symptoms, even after the initial concerns were refuted.

Now, there's a noticeable shift in the conversation. With celebrities using their influence to bring menopause into the spotlight, the so-called ‘Davina effect’, what has that meant for HRT prescribing? Find out more below!

HRT Prescription Trends - What’s the Picture?

Interesting to see the significant increases in HRT prescriptions and notable shifts in prescribing patterns over a long period where we’ve seen much change in attitudes to menopause.

  • HRT prescription boom: In the year 2022-23, a whopping 11 million HRT items were prescribed. That's a massive 47% increase compared to the previous year, 2021-22. It looks like more people are turning to HRT to manage the ups and downs of menopause. Perhaps it's due to the increasing awareness, celebrities opening up about their menopause experiences, and a shift in healthcare practices.

  • More Patients, More Acceptance?: Approximately 2.3 million patients received HRT prescriptions during this period, marking a 29% increase from the year before. This surge in patient numbers suggests that HRT is becoming a more accepted treatment option as awareness spreads and more women seek support from their GP practices.

Is access to HRT fair for everyone?

One eye-opening takeaway from the report is the apparent difference in HRT prescriptions among various socioeconomic groups. The data reveals that more than double the number of patients in the least deprived areas were prescribed HRT compared to those in the most deprived areas. This disparity highlights the continued need for fair access to all women’s healthcare services, as unequal access can lead to unequal health outcomes.

HRT Supply Update

The massive uptick in HRT prescriptions has helped to create a situation where demand outpaced supply. This left people feeling frustrated and concerned. In addition to the increased demand, there have been shortages because of manufacturing disruptions with ingredients being in short supply, and increased regulatory challenges.

Supply issues improved after the creation of the HRT Supply Taskforce in 2022 but there are still issues with several products due to ingredients needed. The British Menopause Society provides ongoing updates on supplies and possible alternatives when products are out of stock.

This report goes beyond mere statistics. While it's clear that HRT isn't suitable for everyone, the goal is to make it more affordable and accessible for those who require, choose, and can safely use it. In the ever-changing landscape of healthcare and menopause awareness, staying informed is crucial for making well-informed decisions.
